Il s’agit de présenter le projet, la place et la centralité de L’essence de la manifestation dans l’œuvre de M. Henry, puisque c’est dans cet ouvrage qu’il a mis au jour les présuppositions de sa phénoménologie de la vie et de l’intériorité subjective, présuppositions qu’il fera travailler dans tous les ouvrages qui lui succéderont, ceci (...) jusqu’à L’incarnation. Elle s’oppose à une phénoménologie du monde, de l’extériorité et de l’objectivité. Son concept central, celui de chair, pense la matière affective du Soi, c’est-à-dire du corps subjectif. Il est inséparable d’une thèse : à savoir que la corporéité (rapport au corps) est antérieure à l’intentionnalité (rapport au monde). Une phénoménologie du corps subjectif est donc apte à saisir la vie dans son immanence. We are skeptical that today’s medical ethics education adequately fosters future physicians’ commitment to beneficence, which is both rationally defensible and fundamentally consistent with patient autonomy. We use an ethical dilemma that was presented to a group of third-year medical students to examine how ethics education might be causing (...) them to give undue deference to autonomy, thereby undermining their commitment to beneficence. INTRODUCTION The right of patients to choose which treatments they prefer is rooted in today’s social mores and taught as a principle of medical ethics as respect for autonomy. Yet, when physicians use their clinical knowledge and skills to advance the well-being of their patients, there may be a conflict between patient autonomy and physician beneficence. We are skeptical that today’s medical ethics education adequately fosters a commitment to beneficence, which is both rationally defensible and fundamentally consistent with patient autonomy. I. An Ethical Dilemma The impetus for this paper arose when students who were completing their third clinical year discussed a real-life ethical dilemma. A middle-aged man developed a pulmonary hemorrhage while on blood thinners for a recently placed coronary stent. The bleeding was felt to be reversible, but the patient needed immediate intubation or he would die. The cardiologist was told that the patient previously expressed to other physicians that he never wanted to be intubated. However, the cardiologist made the decision to intubate the patient anyway, and the patient eventually recovered.[1] Students were asked if they believed that the cardiologist had acted ethically. Their overwhelming response was, “No, the patient should have been allowed to die.” We looked into how students applied ethical reasoning to conclude that this outcome was ethically preferred. To explore how the third-year clinical experience might have formed the students’ judgment, we presented the same case to students who were just beginning their third year. Their responses were essentially uniform in recommending intubation. While there is likely more than one reasonable view in this case, we agree with the physician and the younger medical students that intubation was the ethically appropriate decision and will present an argument for it. But first, we explain the reasoning behind the more advanced medical students’ decision to choose patient autonomy at the expense of beneficence. II. Medical Ethics Education and the Priority of Autonomy Beauchamp and Childress’s Principles of Biomedical Ethics, first published in 1979 and now in its 8th edition, is a significant part of the formal ethics education in medical school.[2] Students learn an ethical decision-making approach based on respect for four ethical principles: autonomy, beneficence, nonmaleficence, and justice. While Beauchamp and Childress officially afford no prima facie superiority to any principle, the importance of respect for patient autonomy has increased through the editions of their book. For example, early editions of their book opposed the legalization of physician-assisted death compared to recent editions that defended it.[3] As another example, Beauchamp and Childress make paternalism harder to justify by adding an autonomy-protecting condition to the list of conditions for acceptable paternalism.[4] Authority, they contend, need not conflict with autonomy—provided the authority is autonomously chosen.[5] “The main requirement,” they write, “is to respect a particular patient’s or subject’s autonomous choices, whatever they may be.[6] In the principlism of Beauchamp and Childress, autonomy now seems to have a kind of default priority.[7] However, the bioethics discourse has strong counternarratives, noting some movement to elevate the role of beneficence and to respect the input of stakeholders, including the family and the healthcare team. Ethics education achieves particular relevance in the third clinical year when students become embedded in the care of patients and learn from what has been called the informal curriculum. They observe how attending physicians approach day-to-day ethical problems at the patient’s bedside. In this context, students observe the importance of informed consent for serious treatments or invasive procedures, a practice that highlights the principle of patient autonomy. In both the formal and informal curriculum, medical students observe how, in the words of Paul Wolpe, “patient autonomy has become the central and most powerful principle in ethical decision-making in American medicine.”[8] In short, students appear to learn a deference for patient autonomy. This curricular shift in favor of autonomy coincides with legal developments that protect patients’ rights and decision-making with respect to their healthcare choices. The priority of autonomy in medicine benefits patients by reflecting their choices and, in some cases, their fundamental liberty. III. The Practice of Medicine and the Commitment to Beneficence There are many critiques of the dominant place that autonomy has in biomedical ethics,[9] especially considering that autonomy seems to be biased toward individualistic, Western, and somewhat American culture-driven values.[10] In addition, many bioethical dilemmas are cast as a conflict between autonomy and beneficence. Our point is that medical students bring to their study of medicine a commitment to beneficence that seems to be suppressed by practical ethics education. We think this commitment is rationally defensible and should be nurtured. It is striking that young medical students have a pre-reflective commitment to beneficence at all. For, as we mentioned, it is not just medicine but Western culture generally that prioritizes autonomy in settling ethical dilemmas. In wanting to act for the good of others (rather than simply agreeing to what others want), physicians are already swimming somewhat against the cultural tide.[11] However, doing so makes sense, given the nature of medicine and the profession of healing. When prospective medical students are asked why they wish to become physicians, the usual answer is some variation on caring for the sick and preventing disease. It is unlikely that a reason to become a physician is to respect a patient’s autonomy. It would be easy to dismiss medical students’ commitment to beneficence as a mere intuition and contrary to a more reasoned and deliberative approach. Beauchamp and Childress seem to minimize the value of physician intuition, stating that justifications for certain procedures are “…supported by good reasons. They need not rest merely in intuition or feeling.”[12] Henry Richardson writes that “situational or perceptive intuition…leaves the reasons for decision unarticulated.”[13] We think this is a crude and rather thin way of understanding intuition. Some bioethicists have defended intuition as essential to the practice of medicine and not something opposed to reason.[14] In the case we describe, we believe the ethical justifications s for the patient’s intubation are fundamentally sound: the patient did not have a “do not intubate” order written in the chart, the emergency intubation had not been foreseen, so the patient did not have the opportunity to consent to or reject intubation; the patient had consented to the treatment for his cardiac disease so his consent for intubation could have been assumed;[15] and the consequences of respecting his autonomy did not justify allowing him to die.[16] While it is possible to have more than one reasonable view on this case, we think the case for beneficence is strong and certainly should not be dismissed out of hand. We do not deny that if a patient makes a clearly documented, well-informed decision to forgo intubation that this decision ought to be respected by the physician (even if the physician disagrees with the patient’s decision). But, in this situation, as in many others in the practice of medicine, the patient’s real wishes and preferences are not well-articulated in advance. There are many cases where a physician acts based on what she believes the patient, or the surrogate, would want, sometimes in situations that do not allow much time for reflection. An example might be resuscitation of a newborn at the borderline of viability. In their ethics education, beneficence would mean acting first to save a life. If the patient or surrogate makes an informed decision to the contrary, a beneficent physician respects that autonomous decision. In the case presented, the patient expressed gratitude to the cardiologist when extubated. But what if he had expressed anger at the physician for violating his autonomy? There are those who could argue that not only was intubation ethically wrong but that the cardiologist put himself in legal jeopardy by his actions (especially if there had been a written refusal applicable to the specific situation). In the example we use, we point out that the cardiologist may not have escaped a lawsuit if the patient had died without intubation. His family, when hearing the circumstances, may have sued for failure to act and dereliction of the cardiologist’s duty to save him. Beyond a potential legal challenge for either action or inaction, there is an overriding ethical question the cardiologist had to address: what course would be most satisfying to his conscience? Would he rather allow a patient to die for fear of recrimination, or act to save his life, regardless of the personal consequences? In the absence of real knowledge about the patient’s considered wishes, it is most reasonable to err on the side of promoting patient well-being. A physician’s commitment to beneficence is not necessarily a way of undermining a patient’s autonomy. In acting for the patient's good, physicians are also acting on what it is reasonable to believe a patient (or most patients, perhaps) would want, which is obviously connected to what a patient does want. Pellegrino and Thomasma argue that beneficence includes respect for a patient’s autonomy since “the best interests of the patient are intimately linked with their preferences.”[17] Instead of conceptualizing ethical dilemmas in medicine as conflicts between autonomy and beneficence, it is possible that medical schools could teach students that truly practicing beneficence is a way of valuing patient autonomy, especially when the patient’s wishes are not specific to the situation and are not clearly expressed. CONCLUSION It is important for students and practicing physicians to understand the principle of respect for patient autonomy in a pluralistic society that demands personal self-determination. However, the role of the physician as a beneficent healer should not be diminished by this respect for autonomy. Respecting a patient’s autonomy is grounded in and manifested by physician beneficence.[18] That is, seeking what is good for the patient can only be good if it respects their personhood and dignity. We propose that a commitment to beneficence, incipient in young medical students, should be developed over time with their other clinical reasoning skills. Such a commitment need not be sacrificed on the altar of patient autonomy. Beneficence needs greater relative moral weight with students as they proceed in their ethics education. - [1] S. When evolutionary explanation is transferred from its normal habitat of biology to the realm of human social, cultural, and moral concerns, a problem is often neglected. After examining arguments for and against Social Darwinism, this paper identifies this problem and proceeds by exploring the possibility of a middle-ground position according to which Social Darwinism would be enough for explaining some aspects of moral and social behaviour but not enough for explaining all aspects. The investigation indicates that this middle-ground position is (...) not viable because intentional content burdens evolutionary ethics with fundamental problems, problems that need to be resolved before any further progress is attainable. While much has been written about Levinas's conception of ethics, very little has been said about the connection between ethics and holiness in his work. Yet, throughout much of his corpus, Levinas consistently links the two. The first part of my article addresses the important distinction that Levinas establishes between the sacred (le sacré) and holiness (la sainteté). According to Levinas, several influential thinkers conflate these two categories. Holiness, Levinas suggests, represents a kind of antidote to the sacred. The second (...) part examines the various ways that holiness is manifest in ethicality. The key, I suggest, for properly understanding the link between ethical regard and sanctification revolves around an appreciation of Levinas's frequent appeal to the notion of straightforwardness (droiture). According to the Appraisal-Tendency Framework (Han, Lerner, & Keltner, 2007), certainty-associated emotions increase risk taking compared with uncertainty-associated emotions. To date, this general effect has only been shown in static judgement and decision-making paradigms; therefore, the present study tested the effect of certainty on risk taking in a sequential decision-making task. We hypothesised that the effect would be reversed due to the kind of processing involved, as certainty is considered to encourage heuristic processing that takes into account the emotional cues (...) arising from previous decisions, whereas uncertainty leads to more systematic processing. One hundred and one female participants were induced to feel one of three emotions (film clips) before performing a decision-making task involving risk (Game of Dice Task; Brand et al., 2005). As expected, the angry and happy participants (certainty-associated emotions) were more likely than the fearful participants (uncertainty-associated emotion) to make safe decisions (vs. risky decisions). Lévinas’s harsh criticisms of the sacred have irked not just his critics but even some who sympathize with his work. Taken at face value, some of Lévinas’s comments concerning the sacred appear prejudicial towards non-monotheistic religions. But a closer reading of his analysis of the sacred shows that his preoccupation with the sacred has to do with a questionable “temptation” or disposition found in every human being. Drawing on the insights of the Bible, Shakespeare, and Lévy-Bruhl, Lévinas shows how this (...) temptation involves a primitive wish to escape one’s responsibilities as a human being. For Lévinas, the phenomenon of the sacred promotes the illusion of a direct and immediate experience of the absolute or the Infinite. Genuine transcendence, for Lévinas, is possible only within the ethical drama of self and Other. The apex of human life involves not the eclipsing of the self—the fundamental motivation behind the sacred—but a heightening of the self’s radical uniqueness that only moral engagement makes possible. A common view in affective neuroscience considers emotions as a multifaceted phenomenon constituted by independent affective and motor components. Such dualistic connotation, obtained by rephrasing the classic Darwin and James’s theories of emotion, leads to the assumption that emotional expression is controlled by motor centers in the anterior cingulate, frontal operculum, and supplementary motor area, whereas emotional experience depends on interoceptive centers in the insula. Recent stimulation studies provide a different perspective. I will outline two sets of findings. First, affective (...) experiences can be elicited also following the stimulation of motor centers. Second, emotional expressions can be elicited by stimulating interoceptive regions. Echoing the original pragmatist theories of emotion, I will make a case for the notion that emotional experience emerges from the integration of sensory and motor signals, encoded in the same functional network. Tool-use behavior is currently one of the most intriguing and widely debated topics in cognitive neuroscience. Different accounts of our ability to use tools have been proposed. In the first part of the paper we review the most prominent interpretations and suggest that none of these accounts, considered in itself, is sufficient to explain tool use. In the second part of the paper we disentangle three different types of reasoning on tools, characterized by a different distribution of motor and cognitive (...) ingredients. At the conceptual level, these types of reasoning reflect the distinction between three types of abductive inference as they are described in semiotic studies. At the functional level, we suggest that these types of reasoning on tools may correspond to different mental processes, possibly implemented in different regions of the left inferior parietal lobe. This proposal can account for the different interpretations commonly associated with the role of the left parietal cortex in tool use. (shrink)

This paper explores how realism is crucial in understanding rule-following. The strategy involves starting from what has been achieved by Wittgenstein and others as regards semantic normativity and then applying it to other areas, including moral deliberation. The result shows that realism in rule-following involves not only the weak claim that rules are independent of the individual rule-follower, as conventions are. It involves also the stronger claim that conventional rules are constrained by non-conventional constraints. These constraints depend neither on the (...) individual nor on the group. They arise from the nature of things. Human rule-followers are constrained first by being material beings, secondly by being living organisms, and thirdly by being rational. (shrink)

BackgroundHigh frequency repetitive transcranial magnetic stimulation of the left dorsolateral prefrontal cortex has shown significant efficiency in the treatment of resistant depression. However in healthy subjects, the effects of rTMS remain unclear.ObjectiveOur aim was to determine the impact of 10 sessions of rTMS applied to the DLPFC on mood and emotion recognition in healthy subjects.DesignIn a randomised double-blind study, 20 subjects received 10 daily sessions of active or sham rTMS. The TMS coil was positioned on the left DLPFC through neuronavigation. (...) Science has uncovered many mistakes that had been hidden for centuries among implicit everyday assumptions. When we make explicit what lies implicit within language, there is no guarantee that we will arrive at truth about the world. Many therefore assume that only science delivers truth. Recent debates on this issue often refer to Wilfred Sellars’s arguments against the pre-conceptual given but conclude that his additional insistence on the exclusivity of the scientific image of the world is unfounded. In this paper (...) I resort to Robert Brandom’s development of these arguments to show that saying a word, understanding a concept and engaging in some practices go together. Both laws of nature and social norms regulate these practices and determine the identity conditions of objects. I argue therefore that the conceptual scheme indeed reflects the nature of things because it results from our successful engagement with the world during the long sweep of evolutionary time. The ventricular pressure profile is characteristic of the cardiac contraction progress and is useful to evaluate the cardiac performance. In this contribution, a tissue-level electromechanical model of the left ventricle is proposed, to assist the interpretation of left ventricular pressure waveforms. The left ventricle has been modeled as an ellipsoid composed of twelve mechano-hydraulic sub-systems. The asynchronous contraction of these twelve myocardial segments has been represented in order to reproduce a realistic pressure profiles. To take into account the different energy (...) domains involved, the tissue-level scale and to facilitate the building of a modular model, multiple formalisms have been used: Bond Graph formalism for the mechano-hydraulic aspects and cellular automata for the electrical activation. An experimental protocol has been defined to acquire ventricular pressure signals from three pigs, with different afterload conditions. Evolutionary Algorithms have been used to identify the model parameters in order to minimize the error between experimental and simulated ventricular pressure signals. Simulation results show that the model is able to reproduce experimental ventricular pressure. In addition, electro-mechanical activation times have been determined in the identification process. For example, the maximum electrical activation time is reached, respectively, 96.5, 139.3 and 131.5 ms for the first, second, and third pigs. These preliminary results are encouraging for the application of the model on non-invasive data like ECG, arterial pressure or myocardial strain. (shrink)

Son contenu reflète les préoccupations féministes de son temps, pour aborder des sujets les plus délicats comme les plus tabous de la société de la fin des années (...) 1970, parmi lesquels la sexualité féminine, l’inceste ou les différentes violences subies par les femmes. Des dossiers consacrés à une thématique particulière sont appuyés par une mise en image sans détours, crue et parfois cruelle. Il en faut beaucoup pour bouleverser les mentalités. Les femmes d’Ah! Nana en ont-elles trop fait? En tout cas ce projet était trop original pour survivre à cette époque. Il est un échec commercial que le poids de la censure condamne définitivement. Mais il reste une tentative, jusqu’ici jamais reconduite en France, de permettre aux femmes de s’exprimer dans la bande dessinée et il rend compte de leurs difficultés à être publiées. (shrink)

Despite considerable debate as to what corporate social responsibility is, consumer social responsibility, as an important force for CSR :19–45, 2005), is a term that remains largely unexplored and under-theorized. To better conceive the role consumers play in activating CSR, this paper provides a multi-level, multi-agent conceptualization of CnSR. Integrating needs-based models of decision making with justice theory, the article interpretively develops the reasons why variously positioned agents leverage consumers as a force for corporate social responsibility. The paper theoretically expands (...) currently limited conceptions of CnSR by exploring the levels at which diverse agents engage with CnSR and the needs driving these agents. The paper suggests that the so-called “consumer side of CSR” is contingent upon the presence, absence, and varying intensities of underlying agent needs. Cet ouvrage explore les critiques du donné dans le néokantisme et la phénoménologie allemands. Il révèle la portée considérable de ces critiques, qui impliquent en même temps une controverse avec le positivisme de la fin du XIXème siècle et un dialogue substantiel avec l’Esthétique transcendantale de Kant. En posant les questions de la sensation et de la perception, la présente monographie permet de ménager un accès privilégié aux enjeux fondamentaux de la philosophie austro-allemande au tournant du XXème siècle. Lorsqu’il est (...) question du donné ou des critiques du donné dans le discours philosophique contemporain, et cela arrive souvent dans les débats sur la perception en philosophie de l’esprit, c’est toujours à la controverse analytique autour du « mythe du donné » que l’on fait référence – controverse, qui a vu le jour en 1956, lorsque le philosophe américain Wilfrid Sellars publie Empirisme et philosophie de l’esprit. Or, il est intéressant d’observer que la critique sellarsienne du « mythe du donné » a été préfigurée, en un sens partiel mais important, par les objections que les néokantiens et les phénoménologues ont adressées à la notion de donné au tournant du XXesiècle. La présente monographie comble cette lacune béante de l’historiographie classique, qui a jusqu’ici accordé peu d’attention à la constance de cette critique dans toutes les philosophies marquées par l’héritage de Kant. Comment, en se basant sur l’ouvrage de Claudia Moatti Res publica. Histoire romaine de la chose publique, dessiner les contours de ce que serait une science historique de l’antiquité qui isole, pour s’en prémunir, les représentations politiques qui nous lient à Rome? On cherche ici à étudier les concepts politiques antiques en plaçant au premier plan les questions de la langue et l’anthropologie des pratiques qu’on essaie de deviner en toile de fond. Ce dossier cherche donc à prolonger l’ouvrage de (...) Claudia Moatti en reprenant l’étude de la Res publica afin de suivre aussi la réception qu’en a fait la postérité. (shrink)

Mais qu'en est-il de la littérature contemporaine? Comment agence-t-elle la mémoire et l'espérance, le " déjà plus " et le " pas encore ", le maintenant et l'autrefois? Quels sont les rythmes et les durées, les passés et les futurs qui fondent la singularité de la littérature du présent, de notre présent? This paper is based on a research conducted between 2004 and 2006 and dealing with the memories of women in a steel valley struck by depression since the seventies, in the North-Eastern part of France. The imagery of steel-producing Lorraine coalesced in a rather standardized way around the figure of the steelworker working at the blast furnace. This research and the exhibition which followed from it, highlighted the activities of women, in the working place as well as in the domestic (...) realm and in community or political movements. It thus contributes to a more complex imagining and less androcentric perspective on the ironworks past of the Lorraine region. (shrink)

Business, management, and business ethics literature pay little attention to the topic of AI robots. The broad spectrum of potential ethical issues pertains to using driverless cars, AI robots in care homes, and in the military, such as Lethal Autonomous Weapon Systems. However, there is a scarcity of in-depth theoretical, methodological, or empirical studies that address these ethical issues, for instance, the impact of morality and where accountability resides in AI robots’ use. To address this dearth, this study offers a (...) conceptual framework that interpretively develops the ethical implications of AI robot applications, drawing on descriptive and normative ethical theory. The new framework elaborates on how the locus of morality and moral intensity combine within context-specific AI robot applications, and how this might influence accountability thinking. Our theorization indicates that in situations of escalating AI agency and situational moral intensity, accountability is widely dispersed between actors and institutions. ‘Accountability clusters’ are outlined to illustrate interrelationships between the locus of morality, moral intensity, and accountability and how these invoke different categorical responses: illegal, immoral, permissible, and supererogatory pertaining to using AI robots. These enable discussion of the ethical implications of using AI robots, and associated accountability challenges for a constellation of actors—from designer, individual/organizational users to the normative and regulative approaches of industrial/governmental bodies and intergovernmental regimes. The majority of evidence on social anxiety -linked attentional biases to threat comes from research using facial expressions. Emotions are, however, communicated through other channels, such as voice. Despite its importance in the interpretation of social cues, emotional prosody processing in SA has been barely explored. This study investigated whether SA is associated with enhanced processing of task-irrelevant angry prosody. Fifty-three participants with high and low SA performed a dichotic listening task in which pairs of male/female voices were presented, one (...) to each ear, with either the same or different prosody. Participants were instructed to focus on either the left or right ear and to identify the speaker’s gender in the attended side. Our main results show that, once attended, task-irrelevant angry prosody elicits greater interference than does neutral prosody. Surprisingly, high socially anxious participants were less prone to distraction from attended-angry prosody than were low socially anxious individuals. These findings emphasise the importance of examining SA-related biases across modalities. (shrink)
